Community Health Partners"Medical Care for Adults and Children" Hours
of Service: Community Health Partners (CHP) is a non-profit medical clinic created to provide affordable, high quality health care to the citizen of Park County. Community Health Partners represents a partnership with all the local health and social serviced agencies, many of which contribute time and money through their involvement. A federal grant helps with approximately 60% of Community Health Partners' costs, while patient revenue, private grants, and donations supply the remainder of the funds. Community Health Partners is available to patients with or without health insurance. CHP fees are discounted to patients who provide proof of income and meet federal poverty guidelines. No one is denied care due to inability to pay at time of service. However, in order to make affordable health care available to everyone, patient revenue is crucial to Community Health Partners operation. As Passport providers, Community Health Partners is open to individuals on Medicaid, as well as Medicare, and any form of private insurance. Services are offered on a "sliding discount schedule", with a minimum payment requested at the time of service. This applies to all medical care provided at Community Health Partners' facility (126 S. Main). Laboratory and x-ray services are available through an arrangement with Livingston Memorial Hospital, with an additional minimum fee charged for these procedures. Prescriptions issued at Community Health Partners are primarily the patient's financial responsibility, though coupons are available to subsidize certain prescription, based on the sliding discount schedule. Obstetrical care is available through and arrangement with the Park Clinic obstetricians.
